Gut Barrier and Immunity

It would be a rather large mistake to discuss nutrition, inflammation and iImmune function without explaining the role of our intestinal lining. Our intestines interact with the external world more than you may be considering - in terms of surface area, our intestines provide more contact with the external world than both the skin or lungs. As a Dietitian, I think about this organ quite a bit and am in awe of the many things it does to keep us alive and healthy. Our intestines (small and large) provide both a physical and chemical barrier to the trillions of pathogens and bacteria that enter our body from the food we eat that is not only full of nutrients but also full of microbes, chemicals and contaminates. Our small intestinal barrier is strongly built with 4 layers that have many cells that secrete peptides, enzymes, hormones all while communicating with our brain and nervous system with the many nerve fibers throughout. It is hard to believe that this 20 foot organ will digest, absorb and move food through while also communicating with endocrine organs to enhance digestion or the brain to communicate fullness or hunger while also protecting our immunity by killing harmful pathogens or preventing an overgrowth of non-beneficial bacteria.


Gut microbiome is a term that refers to the trillions of bacteria that reside in the human gut. The large intestines house the majority of these microbes. The large intestine acts as a major immune organ with abundant lymphoid tissue. Gut microbiome has many important roles: assists with digestion and fermentation of food, creates essential vitamins, and provides the framework for a biofilm that traps pathogens. This protective film along with very tight junctions between intestinal cells is what we refer to as a healthy intestinal barrier. A compromised intestinal barrier can lead to inflammation, hormone imbalance and compromised health.


You can probably imagine, this job is quite messy! Our intestines are exposed to several thousand species of bacteria plus fungi, viruses and more external threats just waiting to multiply and cross over those many intestinal layers to enter our bloodstream and infect us with illness. The intestines have the highest antigen load of any other organ and is essential to our immune system that maintains our safety. The composition of the microbiome is individualized and changes throughout our lifetime. Over the years, research was focused on analyzing fecal microbes for specific species of bacteria or fungi to identify the perfect balance of non-pathogenic species for optimal health with failed results. New research has identified that there may not be an ideal composition for the human population as a whole but is more about the microbes and their host environment that is influenced by many things including food, oxygen availability, tight junctions, mucus linings etc. If the microbes are not properly controlled by the host environment then problems such as chronic inflammation and disease may occur. A simple example of this is when we need to ferment food in our gut which is an anaerobic process (happens in absence of oxygen) yet for some reason, possibly due to a low fiber diet, oxygen leaks into the gut lumen which has the unfortunate effect of causing an influx of detrimental bacteria that is tolerant of oxygen leading to an imbalanced microbiome. This theory indicates that diet and gut environment will influence the actual microbe composition. If there is a predominance of bacteria that requires oxygen to grow then limiting the host environment of oxygen may help balance things. There is a lot of research left in this field before strong recommendations can be made.


So….. what do we know? What does a healthy gut look like?


Global Signs of a Healthy Gut


  • Symptom Free Digestion

    • Minimal gas and bloating after eating

    • Stable bowel movements (daily or every 2 days)

    • Healthy stool consistency (solid texture, not floating)

  • Adequate Absorption

    • No clinical or laboratory signs of vitamin/mineral deficiencies

    • Stools are not floating, no oil on surface

    • Weight is maintained with average caloric intake (if you eat 3000 calories a day and are losing weight without exercise - consider medical evaluation)

  • Healthy Metabolism

    • Glucose levels maintained

    • Wounds heal

    • Blood clots appropriately

    • Hunger and Fullness cues on target

    • Body temperature regulated

  • Low Risk of Infections

    • Recovers appropriately from common illness

    • Frequency of illness is not excessive

  • Mental Health Balanced

    • Depression and Anxiety have been linked to gut health

    • Schizophrenia and Bipolar diseases also have gut microbiome links.

Nutritional Habits and Intestinal Barrier


It is too simple to state that there is a specific diet that is best for the gut microbiome. It varies for each person based on the individual diet consumed, the individual health status including inflammatory illnesses, and current gut environment. What studies have shown specific to diet and intestinal barrier are listed here:


  • High fiber diet is protective for maintaining intestinal barrier

  • Diets high in saturated fats reduce junctions and impair intestinal barrier

  • Diets high in simple sugars may contribute to a leaky gut

  • Optimal Vitamin D status has been shown to support immune system

  • High processed meats has been shown to increase intestinal inflammation

  • Food additives including emulsifiers have been found to increase metabolic syndrome, inflammation, and thin the mucus lining that protects intestinal barrier.


Lifestyle and Intestinal Barrier


In addition to diet, balanced physical activity, adequate sleep and stress management are linked to a healthy balanced gut microbiome which makes sense as our body needs all to be well nourished and recovered in order to handle all of the microbes, toxins and pollutants our gut encounters daily. Poor recovery leads to hormonal imbalance which affects the function of all cells. As we have learned, our gut immune cells and intestinal barrier are extremely important to prevent a leaky gut that allows bacteria to translocate where it does not belong.
